REQUIRED Participation Agreement
All participants (or guardians) are responsible for completing this form prior to engaging in Macdonald programming. This must be completed prior to the first scheduled event. NO EXCEPTIONS! On-line form will be submitted to Ringette Manitoba for tracking purposes. Under Association Affiliation please select Ringette Manitoba.
REQUIRED Daily Attestation Form
All participants (or guardians) are responsible for completing a Daily COVID-19 Attestation. This must be completed within 8 hours of any Macdonald event and will be collected by the Safety Officer assigned to the player's squad. This is currently a printable form but Ringette Manitoba and Macdonald are both pursuing on-line solutions to simplify this process for our members.
Self Assessment Tool COVID-19 Symptoms
Macdonald members will be required to follow all protocols for the venues where we conduct programming. These protocols may vary by venue and can have implications on arrival time, capacities for players and spectators, entry doors, etc. This website will be updated with additional venues as more events are planned. Macdonald Safety Officers will be on-site for each event to ensure protocols are being followed. We thank all members in advance for their support of these volunteers.
